Registration Procedure

In order to register with the practice you must normally be resident in the islands of Yell or Fetlar.

Application and new patient questionnaire forms can be collected from the practice or downloaded below.

Please complete both forms accurately and remember to sign and date the reverse of the application form.

Completed forms can be emailed, posted, or dropped of at the practice.

Once received, your previous GP practice will be contacted to request a summary of your record.

Your full record may take a while to be received by the practice.

If you will need medication prescribed please contact the practice at least two weeks before you run out of any medicines you need.
